For a full life in Ireland, you need to issue several important documents. Here is their list and samples:

1) PPS number.

You have issued a request to receive it upon arrival in the country. Please wait a bit - in 5 working days your PPS number should be available. You can get it from your local intreo office.

Keep in mind that you can start issuing all other documents only if you have a PPS number.

2) Public Services Card.

To receive it, you need to go to Dublin, to the Ukraine Support Center Dublin City.

You must have your PPS, your passport and local phone number (an SMS will be sent to it) and know the exact address of the campus where you live. You will be photographed and asked to fill out a small questionnaire. The completed card will be mailed to your campus within 10 business days.

3) Health Service Executive

Your health insurance. You can download the form for filling out from the link.

The form must be filled in English. Your data must match your documents. The form is filled out one per family or for each adult separately. You can find a sample of filling in the link Section 3, where information about the doctor is filled in, leave it blank. The printed and completed form must be photographed (each page) and emailed to PCRS.Applications@HSE.ie.
